The opening-verse is by way of introduction, but this theme is not devoid of pleasure-- that for the heart to become blood, there was need for more blood. This it obtained from the liver, and in this way the heart destroyed the liver itself.

There's also an especially complex evocation of the 'kya effect', made possible by the sheer, ambiguous simplicity of kyā rahā hai :
